What is an Independent Waiver Program?
Home and community based waiver programs enable individuals to live in a community and remain as independent as possible.
Who qualifies for services?
Individuals between the age of:
18-59 (OLTL Waivers)
60+ (PDCA Waivers)
Individuals with:
physical disabilities
traumatic brain injuries
diabetes
arthritis
chronic asthma
heart/kidney problems
hypertension
cancer
stroke
paraplegia
Parkinson’s disease
MS
Huntington’s disease
developmental physical disabilities such as cerebral palsy, autism, epilepsy or similar conditions
both mentally-art and non-mentally alert
Who is eligible for the program?:
Individuals must:
Be a resident of Pennsylvania
Be a citizen of the United States or immigrant lawfully admitted for permanent residence
Have medically determinable impairment which can be expected to last at least 12 months
Because of the disability, need help to complete daily living tasks, self-care, understanding and use of language, learning, mobility, self-direction and/or capacity for independent living
Have income limit not to exceed $2,022 (monthly gross)
